Christina Noble is a child of the Dublin slums. Her story is one of bravery and resilience in the face of deprivation and abuse. After the early death of her mother her family was split apart, their alcoholic father unable to care for them. Christina was sexually abused and later escaped from an orphanage only to become destitute on the streets of Dublin. At 16 she was pulled into a car by four men and raped repeatedly. Later driven to near insanity by overwork and a violent husband, she found in a dream the will to fight back and her hope lay in a determination to work among the bui doi, the street children of Vietnam.

Fears of the prospect of growing social exclusion have become important concerns in recent years for many countries. Improving the quality of education and the standards attained by students to improve employability is one of the tools being used to prevent exclusion. However, changing social realities are leaving young children and students more exposed than ever to failure at school and unemployment. It is becoming increasingly clear that communities, education systems, schools and teachers are not equipped to deal with the many problems which arise and when social or health services become involved conflicts of interest can arise leading to actions which are not always in the clients' best interests. The necessity to provide greater co-ordination among these services, to improve their efficiency and effectiveness and to provide a seamless support to meet the holistic needs of students and their families is now becoming more accepted. Such an approach, inter alia, is community-based, emphasises prevention rather than being crisis-oriented, is customer-driven rather than being focused on an agency, and is accountable through outcomes rather than inputs. For many, better co-ordination of services is seen as the only solution available which is commensurate with our present democratic societies. All of the papers in this book were presented at a conference held in Toronto, Canada. They are original and have been written by policy-makers from different ministries, researchers from different disciplines and clients who come variously from Canada, Finland, France, Germany, the Netherlands, Spain, the UK, and the USA.

Korea faces an extraordinary ageing challenge. Korea will age much faster than other OECD countries: in 2000, about 7% of Korea’s population were over 65; in 2050, senior citizens will constitute about 37% of Korea’s population. Population ageing will unfold at high speed, firstly because of the dramatic increase in life expectancy from just over 52 years in 1960 to over 77 years in 2004. a major cause of this was the spectacular decrease in infant mortality rates from 45 infants per 1000 live births in 1970 to 5.3 in 2002. The second cause of population ageing is the sharp decline in birth rates from close to 3 children per woman in 1975 to less than 1.2 in 2004. Reduced child mortality rates and increased life expectancy are indicators of the success of the Korean economy and society. However, the decline in fertility rates in Korea is evidence of strains in society which will damage prosperity in the future. Population ageing on this scale will inevitably lead to a huge increase in spending on old-age income support and health care, and will also require the development of a public family policy which supports the reconciliation of work and care commitments of workers. To successfully meet the ageing challenge, the OECD believes that three policy objectives must be targeted. First, the decline in the working-age population needs to be slowed. Second, working opportunities need to be extended. Third, affordable and sustainable pension and health care policies must be implemented. This volume looks at existing Korean family, health and pension policies from an international perspective, considers them in view of the emerging policy challenges, and outlines some of the policy options that are available to policy makers in Korea.

Plus D’un Quart De La Population Russe Est Frappé Par La Pauvreté Au Regard Même Des Critères Officiels. Le Salaire Moyen A Pratiquemment Diminué De Moitié Par Rapport à Son Niveau Réel Du Début Des Années 90 Et Les Retards De Paiement Des Salaires Et Des Pensions Ont Posé De Sérieux Problèmes Il N’y A Pas Si Longtemps. Quant à L’économie Informelle, Elle S’est Considérablement Développée. Que Peut-on Faire Pour Résoudre Cette Crise ? As we move into the 21st century, the turbulent transformation of economy and society looks set to continue. Growing integration of markets, radical new technologies, the increasing knowledge intensity of human activity, all point to the emergence of an immensely complex world. But how will it be managed? And by whom? What forms of organisation and decision-making will be required at local, national and global levels to meet the challenges of the next decades? One thing seems certain: old forms of governance -- in the public sector, corporations and civil society -- are becoming increasingly ineffective. New forms of governance will be needed over the next few decades which will involve a much broader range of active players. Traditional hierarchical organisations and top-down control will give way more and more to a wider diffusion of responsibility and decision-making that builds on the talents for innovation and creativity of individuals and groups. Are breast cancer survival rates higher in the United States than in the United Kingdom and France? Are a patient's chances of dying within 30 days after admission to a hospital with a heart attack lower in Canada than in Korea? Are surgeons in some countries more likely to leave "foreign bodies" behind after operations or make accidental punctures or lacerations rates when performing surgery? The need for answers to these kinds of questions and the value of measuring the quality of health care are among the issues addressed in this publication. Many health policies depend on our ability to measure the quality of care accurately. Governments want to increase "patient-centeredness", improve co-ordination of care, and pay providers of high-quality care more than those who underperform. However, measuring the quality of health care is challenging. The OECD’s Health Care Quality Indicator project has overcome some of the problems, though many remain. If policy makers are serious about improving the body of evidence on the quality of care, they need to improve their health information systems. This publication describes what international comparable quality measures are currently available and how to link these measures to quality policies such as accreditation, practice guidelines, pay-for-performance, national safety programmes and quality reporting.

Trade liberalisation is necessary but not sufficient to provide the conditions under which migration flows are significantly reduced. Co-ordinated measures must therefore be undertaken to promote technological catch-up, the development of physical infrastructure and investment in human capital. Against a background of sustained development accompanied by significant employment creation, the incentive to emigrate could decline. This publication explores the links between trade liberalisation and migration movements in North America and discusses the issue of whether the free circulation of persons accompany the successive stages of regional economic integration or whether it is an important objective to be achieved only once economic convergence has reached a sufficiently high level. The authors conclude that restrictions on movement are likely to continue into the future and that the lack of free movement does not constitute an obstacle to regional economic integration. Within this context, the need for balanced migration policies which take account of concerns about the economic consequences and social welfare of both sending and receiving countries will remain on the agenda for the foreseeable future.
